The MDRD (Modification of Diet in Renal Disease) formula estimates glomerular filtration rate (GFR) from serum creatinine. CKD stages: G1 =90 (normal), G2 60�89 (mildly decreased), G3a 45�59, G3b 30�44, G4 15�29, G5 <15 (kidney failure). Note: MDRD may underestimate GFR at higher levels; CKD-EPI is preferred for eGFR >60.

🏆Pro tip

MDRD is validated for creatinine-stable CKD patients. Do not use in AKI, pregnancy, extremes of muscle mass (bodybuilders, amputees), or malnutrition. For drug dosing (e.g., renally-cleared antibiotics, DOACs, metformin), use absolute GFR = eGFR � BSA/1.73.
